The national strategic direction in General Practice (within Primary Care Networks) is toemploy via the Additional Roles Reimbursement Scheme (ARRS) a wide range of health andcare professionals to manage the increasing demand via the ARRS scheme.
As part of this national scheme, we are looking to recruit older adult mental health link workers(social prescribers) to work with adults over the age of 65 with functional mental health issuesacross all Wigan Borough PCNs with the ambition that it will support joint working with localmental health services and improve access to appropriate community services.
In this role, you will support older adults within a primary care network setting to enhance theirhealth, wellbeing, and quality of life, assisting them to connect with and access mental healthservices, local authority services, voluntary sector services and community-based activitiesand support. The role will be delivered through working as an integral part of both primary andsecondary care services, to connect individuals to appropriate support at the earliestopportunity.
You will be supported by members of the local GP practice, wider primary care network andLiving Well team. CPD opportunities are available and personal interests/skills will besupported to develop and grow. You will work alongside adult mental health link workerandcommunity link workers involved in social prescribing within the Primary Care Network.
