This exciting opportunity is to join the newly formed Urgent Community Response (UCR) Team within the South Tees locality.
Urgent Community Response is a Nationally mandated model of care where patients who require it are assessed and treated within 2 hours of referral. The Urgent Community Response Team is an integrated service which incorporates UCR Nursing, UCR Therapies, Home first and the Virtual Ward.
The aim of the UCR service is to assess and treat patients within the community to prevent avoidable admissions, expedite sustainable hospital discharges and support safety being maintained within patients homes.
The successful post holder will be nominated to predominately work within the therapy branch of the UCR service. However, there will also be an expectation to work collaboratively across the whole of the UCR service as needed. As a Therapy Assistant your role will involve completing specified interventions under the direction of a UCR Therapist. Previous experience of the therapy assistant role would be of benefit, however training can be provided.
This is an opportunity for those who are interested in working in the local community and being part of an exciting new model of working in the community localities of Middlesbrough, Redcar & Cleveland.
You will need to be able to work on your own, as part of a team and be involved with individuals with a range of medical, social and mental health conditions, therefore knowledge and experience of this is needed. You will work within agreed protocols and carry out therapeutic interventions and care programmes under the supervision of qualified therapists and nurses.
Excellent communication skills, organisational skills, enthusiasm, ability to travel around the locality, commitment and ability to work with a wide range of agencies are also vital in this post.
This is an opportunity for anyone with previous experience working within nursing, therapy or in care settings to join our established team. You will assist in providing interventions to patients within their own home preventing avoidable hospital admissions and respond urgently to aim to keep patients in our care safe in the community. As a Therapy Assistant you will work with an integrated approach across all of the UCR service streams including Home first, Virtual ward, UCR Nursing and UCR therapy providing where appropriate rehabilitation and care intervention following a person centred plan within the patients’ own home.
