About the Service/Department:

Hounslow LIFE Integrated Support Service is an exciting service that supports adults at risk of losing their independence through the delivery of personalised preventative support in the community. The service delivers in the areas of: Floating Support, Care Act Advocacy, Carers short breaks and LIFE Wellbeing.

The service operate across the entire borough of Hounslow and provides a seamless service across all elements. We provide practical, person-centred support that promotes wellbeing, independence, dignity and respect. Working with people to identify their strengths, personal goals and aspirations and develop holistic, outcomes-focussed plans to support people as they move towards those goals.

We empower people to speak up for themselves, understand their rights and have greater control over their lives and provide carers with the opportunity to take time out from their caring responsibilities to look after their own wellbeing. We will also work alongside befrienders, volunteers and peer supporters to deliver activities that promote independence and reduce reliance on services

The Role:

Under the direction of the Service Manager, to empower a caseload of clients with mental health conditions to maintain their independence, identify their strengths and achieve their goals and aspirations, working in partnership with our subcontractors as required in order to achieve this. You will act as team expert and lead officer in supporting this client group, supporting colleagues with their own caseloads as required and monitoring the client management system for quality of support to this client group. You will also:

  • Work with the service manage to identify service development opportunity for this client group drawn from gaps in service provision
  • Attend meetings, build networks and partnerships across Hestia, LBH and beyond as appropriate, identifying, developing and sharing best practice for working with this client group
  • Support service users in their liaisons with professionals within the community such as social work teams, housing providers, health professionals, JCP etc.
  • Work flexibly as part of a team to ensure service coverage and that contractual obligations are met
  • Develop community-based approaches to empowering service users to achieve their goals and aspirations and to encourage service users to access these opportunities
  • Actively monitor the health, wellbeing, safety and security of the service and its users ensuring all concerns are followed up promptly and in line with procedures
  • Participate in the core work of the service, providing support to colleagues, carrying out tasks allocated by the team leader and service manager and using initiative to identify and deal with additional tasks as they arise

Personal Specification:

  • Extensive experience of working with people who have a range mental health conditions and other long-term conditions in a person-centred way
  • A working knowledge of relevant legislation such as the Care Act 2014, Mental Capacity Act, Mental Health Act and welfare benefits / housing legislation
  • Ability to act as a leader; supporting colleagues, networking and building expertise and best practice within the team for your specialism
  • Thorough understanding of discrimination and disability issues, equal opportunities and anti- discriminatory practice
  • Ability to promote the service and give presentations to a variety of audiences
  • Ability and willingness to lone work
  • Ability and willingness to work remotely and in the community, using mobile technology
  • Excellent IT skills
  • Excellent written and oral communication skills and attention to detail
  • Knowledge and understanding of safeguarding issues and ability to address them appropriately
  • Ability to design and deliver training
  • Skills for Care Certificate (desirable or willing to work towards)
  • Relevant health or social care qualification and/or training
