We are looking to recruit two Service Managers to lead the teams who work within our Living and Ageing Well Service and in our Adult Future Options Services.
Brief Description for Ageing Well:-
Working with older adults with eligible social care needs including those with dementia living in the community
- Includes people living in extra care and OPIL schemes
- Aligned to Primary Care Network areas
- Focus on continuity of care for a person but teams will work together to manage demand and will be flexible to support each other
- Co-located with one of the Response and Enablement teams
- Community teams will manage safeguarding issues within the community cohort
Working closely with commissioning, brokerage, Social Care Accounting Service, Care Homes teams, and other colleagues as well as external colleagues including health and homecare providers
Brief Description for Adults Future Options:-
- All case management for those of working age, except dementia - includes learning disabilities, physical disabilities, social needs.
- Case management for older people with a learning disability unless their needs are better met through Living and Ageing Well Service.
- Working with people living in the community
- Focused around promoting and enabling adults to live as independently as possible and be connected to their own communities.
- Strong links with health colleagues including CLDT and providers
- Community teams will manage safeguarding issues
- Duty will be the responsibility of the teams, rota basis to be designed with the team.
We are looking for people who are demonstrably excellent managers of teams with first rate leadership skills and abilities. The successful applicant will have a track record of motivating and supporting their teams to work successfully in person centred ways, have experience of problem solving and be able to demonstrate how they have found creative solutions to complex partnership issues both internally and externally to the organisation. The post holder will also need to initiate projects, contribute to wider initiatives and lead aspects of change in the Service. This is a challenging post, but with many opportunities to make a difference to people at what is often a very difficult time in their lives.
