Job description

Looking after patients with CKD 4-5 declining renal function

alongside the Renal consultant and MDT members.

Working across East & West Berkshire

Own caseload

Looking after the patient in a renal holistic way - including renal anaemia management

Autonomous role /plus working as part of a small team with a wider Home therapy team, within the whole Renal unit within the trust.

Providing Patient and their families with Education about Renal Failure, how to slow the decline, treatment options, assisting the in making clinical decisions when the time comes for Renal Replacement therapy.

Working both on the RBH site as the main hub / off site clinics and visiting the patients in their own home s for education and assessments.

Educator of patients with declining kidney function - supporting the patient and their families with the decision making re treatment options as their renal function declines towards Renal replacement.

Own patient case load - monitoring patient

Supporting the Clinician in the support and care of the patient

We are a Monday to Friday core hour service ,the majority of the time. Occasionally the day runs over because of clinic need i.e. clinics or home visits overrun

We deliver Low clearance care to patients across East & West Berkshire - taking the care to our off site renal hubs or the patients home whichever is deemed appropriate for the patient.

We have a flexible working ethic as long as the service is covered and fait to all team members.

We also cross work into the Renal Home therapy team - undertaking patient clinical assessments, Undertaking Iron infusions , face to face patient clinical reviews autonomous role - ability to work alone, to be self -motivated plus being a team member within Kidney care while also being a team member within the wider Renal multi- disciplinary team.

Talent will be encouraged, promoting a confident independent nurse who will become an expert in Kidney care

Patient education around Renal failure/ healthy kidneys / kidney decline / treatment options

Ensuing patients are well informed so they can make informed decisions about their care / supporting them and their families along this renal journey

Patient monitoring both alongside the Renal Consultant plus autonomously

Own case load

Essential to be a car owner driver so the service goes to the patient.

Working also alongside and within the wider Renal Home Therapy team which includes CAPD / Transplant / Renal remote monitoring
