Main Duties and Responsibilities CLINICAL To lead, co-coordinate and continually develop a specialist and evidence based complex wound service with specific responsibility for the improvement of leg ulcer management in order to deliver evidence based care and meet clinical governance requirements. Taking a leading role in service development and evaluation including audit and research. To provide expert knowledge, clinical leadership to the nursing and administrative staff within the Tissue Viability team in the absence of the Tissue Viability lead nurse. Provide clinical leadership and managerial responsibility to a newly configured team of community based nurses with a specialist interest in complex wounds and leg ulcer management.
To provide specialist advice and education to all health care professions throughout the Borough of Central London, Hammersmith & Fulham and West London to ensure high standards of clinical practice. To act as an autonomous practitioner within The trust providing links and close liaison to primary and secondary care as well as independent nursing homes. To assist the Tissue Viability lead nurse to support the clinical development of acute hospital and community nurses in developing wound care skills and knowledge. COMMUNICATION AND WORKING RELATIONSHIPS To have an awareness of potential barriers to communication and an understanding and how to overcome these barriers to ensure effective communication occurs.
To provide positive, effective and clear communication and evidenced based advice to all staffs, patients and carers. To be involved with service promotion, presentations and facilitate patient focused meetings when necessary. To liaise with other services i.e. Vascular, Dermatology, Infection control and Lymphoedema To ensure communication is maintained within the team at all levels.
Recognise the importance of documentation as a form of communication to provide continuity of care for patients and professional accountability. To ensure that shared information is done so in a manner that is in line with maintaining patient confidentiality. ADMINISTRATIVE To ensure that timely and accurate data collection takes place, as required by the trust. To efficiently record patient information on RIO database.
To attend and participate in relevant meetings. To report and document on the trust Datix system any incident or accident to a patient or member of staff, and with the support of senior colleagues take appropriate action. To be aware of and act in accordance with Trust Operational Policies. This includes Health and Safety at Work policy and the Complaints policy.
To be responsible for the care and safe keeping of equipment and to report any defect or loss. To write reports in Word, access/send emails on the Trust system, access the intranet/internet for relevant information according to Trust policy. PERSONAL AND PROFESSIONAL To participate in self-appraisal and performance review, maintaining an up-to-date Personal Development Portfolio. To maintain appropriate and up to date knowledge and skills and undertake continuing education in accordance with personal and service needs.
To be involved in the development of the role of nursing and service delivery within primary care To provide care within the clinical governance framework. FINANCE/RESOURCES To ensure efficient use of resources by promoting adherance to the wound care formulary. To ensure efficient use of medical devices including safe and correct use of equipment including pressure relieving mattress, cushions, topical negative pressure and MIST therapy. RESEARCH AND DEVELOPMENT Participate in audit and research projects carried out by the Tissue Viability team.
Undertake, where appropriate, nursing research with published outcomes. Update own knowledge to promote excellence in clinical practice, ensuring practice is based on contemporary evidence.
