This role requires someone who is motivated and passionate about personalised care. Working in a person centred way, holistically to identify people’s needs and creatively using a Personal Health budget to fulfil their potential, their way.

The post will involve daily working with people using our service, to co-produce personalised care and support plans, based on their previously identified clinical need. The post holder will be the first point of contact for people in relation to their PHB for assessment and planning, ensuring that the administrative process is efficient and effective.
To work as part of the Personality Disorder hub and closely with partners in the Integrated Care Board, Local authority’s and other key stakeholders to ensure the seamless delivery of Personal Health Budgets (PHB) specifically for those in receipt of secondary mental health care services with complex emotional needs.
This role will be instrumental in developing and piloting the personalised care agenda for this population in Nottinghamshire.
This is new role and expected to develop alongside the pilot, therefore role and duties may change in line with service requirements.
Delivery of this role will be in line with local and national PHB Policies.

- Be able to develop relationships with people to aid the personal health budget process. Seeing people in their own homes or the team base.
- Open to developing a sound knowledge of resources available for support planning.
- Involve people in decisions about their own health and care, based on what is best for them.
- Support people and give them choice and control in how to manage their own health, care, and well-being.
- Act as the central point of contact for an allocated PHB case load ensuring that all correspondence, messages and enquiries are logged, acknowledged and prioritised. Ensure those requiring actions are prioritised and communications are responded to appropriately and in a timely and professional fashion.
- Support and maintain an effective and efficient PHB referral management system.
- Facilitate appropriate care planning meetings / appointments with all relevant parties. Use judgement and analysis for the completion of costed support plans and reviews. Ensuring all documentation is collated and prepared for the clinical team and the service users.
