Job Title: Patient Care Navigator – GP Receptionist (2 posts).

(Please note you may be required to cover a variety of functions within the surgery, subject to business needs)

Reports To: Admin Services Supervisor/Reception & Rota Co-Ordinator

Hours: 25-30 Hours Per Week

Job Summary:

  • To understand and implement the care navigation pathway for patients, to ensure they access the appropriate service or healthcare professional in a courteous, efficient, and effective way.
  • To be responsible for a wide range of reception duties and to act as the central point of contact for patients.
  • To provide a range of administrative support and services to all members of the practice team.
  • To project a positive, helpful, and friendly image to patients, staff, and other visitors, either in person, writing or via the telephone.
  • To ensure confidentiality at all times.
  • To use IT, software packages and equipment safely and effectively as required to complete your duties efficiently.
  • To work in accordance with written protocols and ensure all tasks are completed accurately and in a timely manner.
  • To undertake any other tasks allocated by Line Managers.
  • Adhere to practice policies and procedures including quality and diversity, H&S, confidentiality and GDPR.

Main Duties/Key Tasks:
You will initially undertake reception duties but can be asked to undertake any of the following functions to meet the needs of the business, in line with your skills and knowledge. Training will be provided as necessary.
1. Reception
2. Prescriptions
3. Preparation of insurance reports
4. Summarising of patient notes
5. Secretarial duties
6. Dealing with LTC (Long Term Condition) patients
7. Scanning and workflow

Reception Duties

  • Carrying out start/end of day processes as per practice protocols.
  • Offering appropriate care navigation for all patients and visitors as required.
  • Answer incoming phone calls promptly and politely, dealing with the call efficiently and appropriately.
  • Greeting patients and visitors who attend the surgery.
  • Manage appointment requests/cancellations from patients and ensure records are updated promptly with relevant information.
  • To regularly review uncollected prescriptions and destroy as necessary and update patient records accordingly
  • Accurately record prescriptions out to pharmacies
  • Process new patient registration forms.
  • Process patient change of address requests.
  • Scanning of letters and relevant correspondence into patient’s notes.
  • Ensure patient areas are kept clean, tidy and free from hazards.
  • Ensure building security

Competencies:

  • IT literate
  • Sound knowledge of Windows and MS Office
  • Sound knowledge of the practice clinical computer system
  • Sound knowledge of GDPR and confidentiality guidelines
  • Good verbal and written communication skills
  • Problem solving skills
  • Flexible approach, team player, good sense of humour, punctual.

Experience and Qualifications
Essential criteria

  • A minimum of 5 GCSE's or equivalent.
  • Excellent communication skills.
  • High standards of numeracy and literacy.
  • Strong IT skills. Attention to detail with a high level of accuracy.
  • Working under pressure within a busy working environment.
  • The ability to work on your own initiative and as part of a team.
  • To undertake training as required.
  • This post is subject to a DBS check.

Desirable criteria Ability to communicate in Welsh

  • Previous experience within a GP surgery/medical setting.
  • Knowledge/experience of the VISION clinical

The post-holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in the practice Employee Safety Handbook and published procedures. This will include:

  • Using personal security systems within the workplace according to practice guidelines
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
  • Making effective use of training to update knowledge and skills
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
  • Actively reporting of health and safety hazards and infection hazards immediately when recognised
  • Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holder’s role
  • Undertaking periodic infection control training (minimum annually)
  • Reporting potential risks identified
  • Demonstrate due regard for safeguarding and promoting the welfare of children.

The post-holder will:

  • Apply practice policies, standards, and guidance
  • Discuss with other members of the team how the policies, standards and guidelines will affect own work
  • Participate in audit where appropriate

Job Types: Part-time, Permanent
