This post is based in the Service Access team within our H&SCP Intervention Services. Reporting to the Team Manager, You will be involved in “Contributing to specific Occupational Therapy assessments with individuals in the community. This post is an Early Intervention and Preventative Post with a view to promoting independence and personal development, through the continuation of rehabilitation and provision of equipment and adaptations. The successful candidate would be involved in undertaking initial assessments and providing appropriate advice/equipment as necessary to enable people to maintain their independence within their own homes. The successful candidate will also liaise with the wider multi-disciplinary teams to ensure all appropriate services are provided.
