What you will be doing in this role:
Cedars-Sinai COACH (Community Outreach Assistance for Children’s Health) for Kids provides medically underserved children and families with transitional medical care and social support services throughout the South Los Angeles community. After mobile clinic services have been provided, the primary goal is to directly connect families to a permanent source of medical care—a “medical home”—in each family’s neighborhood.
The Community Health Worker supports patients and their social and health needs by helping them navigate and access community resources, social services, and medical systems and connection process to a medical, dental, or vision provider. Serves as a liaison between Cedars-Sinai and the community to facilitate access to services through assessment, care coordination, and other case management activities. The Community Health Worker is community based but will often partner cross-functionally with Cedars-Sinai team members. This includes medical providers, social workers, and case managers to provide intensive personal support to patients to ensure that all health needs are met.
- Assesses social determinant of health needs in patients and documents appropriately.
- Determines social determinants of health concerns/ gaps, develops a plan to address moderate social and health disparities.
- Assists patients with problem-solving barriers to health stabilization by identifying, locating, connecting to, and navigating needed community and medical system services.
Requirements:
- High school diploma/GED required. Bachelor's Degree in Social Work, Psychology, Public Health, Community Health or a related field is preferred.
- Cardiopulmonary Resuscitation (CPR) Heart Saver CPR Certification is required for mobile clinic/COACH for Kids Program.
- Community Health Worker Certification is preferred.
- A minimum of six months of experience in community-level health education or related field is required. Two years of experience working with vulnerable populations, homeless individuals/families, people with mental illness and substance use disorders is preferred.
- Certified Nursing Assistant (CNA) preferred
- Front Office/Outpatient Clinic experience preferred
- Bilingual Spanish/English a plus
