To risk assess all patients and carryout manual handling as per Trust policy.
To maintain accurate patient notes in line with Trust policy.
To ensure admissions processes are correctly adhered to in accordance to Trust guidance, to ensure that the patient pathway is correct and timely. Act as a contact point for queries and enquiries on POA appointments and elective admissions for clinical staff and patients.
Ensure the telephone is answered promptly and efficiently so that all calls are dealt with in a timely and courteous manner. To check answer phone regularly and messages followed up promptly.
Facilitate the booking of POA appointments on IPM and maintain and update clinic working diary on a daily basis.
Regenerate re-booked appointments when necessary, informing nursing staff, consultant secretaries, waiting list office, medical staff as appropriate. Also informing patient in writing and where necessary telephone follow-up.
Facilitate and expedite when advised by nursing staff/clinicians the referral appointments, investigation and clinical reports of patients thus ensuring a smooth and timely patient journey.
To ensure that all patient documentation and reporting from the POA is filed appropriately.
To undertake any other duties that may be deemed reasonable.
To implement planned up to date clinical and personal care under the supervision of the nurse in charge
To carry out routine observations, records ECG’s as required and take and analyse routine urine specimens.
Perform venepuncture at request of Registered Nurse.
To undertake pre-admission screening calls passing any patients of concern to a Registered Nurse
To work with the new triage documentation and evaluating the questions either triaging the patients home with admission information or booking to see a Nurse using the format in the questionnaire
To promptly report changes in clinical condition of patients to qualified staff.
To inform Nurse in Charge of any matters affecting the standard of patient care delivery.
To support other team members in their professional work and to work as part of the nursing team to promote high standards of care.
To be aware of current issues and policies within the Trust and the department and adhere to them.
Contribute to the nursing team in the assessment, planning, implementation and evaluation of programmes of care in line with best practice.
Participate as part of the nursing team in implementing change and service developments.
To assist in ordering and maintaining stock levels of consumables

All new staff will be subject to a probationary period covering their first six months in post
You will at all times work under the supervision of the Registered Nurse in charge of the shift and will be accountable to the Registered Nurse you are working with and to the Senior Sister/Charge Nurse/ of the ward/department
You will work to the level of your experience, assessed competence and skills acquired within the post.
You will require immediate and direct supervision in areas of unfamiliar work. You should not undertake any task that you are unfamiliar with.
You will be required to be rostered in the Preassessment Hub and work with the Registered Nurse to check results and answer any phone calls to the Hub Phone line and pass any messages to the Registered Nurse.
You will be required to be rostered onto Surgical Arrivals – admitting patients for surgery and maintaining the standards of patient care required.
