Family Carer Link Worker - Early Intervention Team (EIT)
£25,408 pro rata per annum (£12,703 for 18.5 hours per week)
Fixed term until 31st March 2025
Based in West Suffolk Hospital with some nearby/remote working and home working
About the role:
An exciting opportunity has arisen to join Suffolk Family Carers as a Family Carer Link Worker based within the Early Intervention Team at West Suffolk Hospital.
Working alongside health and social care colleagues, this role will help to identify and support family carers, providing them with all the information, advice, guidance and emotional support they need to enable them to maintain their own wellbeing alongside that of the person they care for.
This is a part time position of 18.5 hours a week on a fixed term contract until March 2025. This a hybrid role, with time split between the EIT Office within West Suffolk Hospital, home working and out within the local community on home visits.
As a Family Carer Link Worker, you will be responsible for:
- Assessing the needs of the family carer and taking a solution-focused approach
- Providing relevant and up-to-date information, advice, guidance and support to family carers on a one to one basis in their own homes, via the telephone or suitable environment
- Liaising with other professionals to ensure any identified needs are met
- Promoting general health and wellbeing to family carers and identify self-management tools to help to prevent a breakdown of their health and ability to care
- Engaging with wider members of the Early Intervention Team to raise awareness of family carers issues and support the team in a visible and proactive manner

- Strong communication skills
- Calm and empathetic nature
- Understanding of family carer issues and their rights
- Knowledge of local community services
- Able to work independently and as part of a team
- Health coaching / counselling skills
- Ability to prioritise your own workload
This role will be subject to an enhanced DBS check, which will be carried out following acceptance of any offer made to the successful candidate. This is paid for by the organisation.
