The Community Response Team support discharges from the acute hospitals, providing short-term, interim care to people in their own homes.

Previous caring experience is ESSENTIAL.

We have posts available in Amber valley based at Babington Hospital and Chesterfield/North East Derbyshire based at Walton Hospital.

You will need to be flexible and committed to caring and supporting people at home with care needs.

You will need to meet the travel requirements of the post.

The shifts will be either 08:00-14:00 or 16:00-22:00; on a 7 day rota.

36 hours is full time hours there are also part time hours available.

You will be supported and receive training to help you develop.

The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.

To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.

To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the wider multi-disciplinary team.

To carry out individual care without direct supervision in the community or clinic setting.

To provide support to patients to optimise their health and independence.

The Purpose and Activities of DCHS?

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
