PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Villas on The Park Team as the Case Manager II at the San Jose Office.
ABOUT OUR VILLAS ON THE PARK TEAM
Working as part of a multi-disciplinary team, this position is responsible for providing outreach, field based and residential case management, mental health services, and housing location and retention services to clients enrolled in the program.
** CURRENT SIGN ON BONUS : $2,500**
WHAT IS A CASE MANAGER
As part of the Villas on the Park Team, the Case Manager performs housing and supportive services to chronically homeless individuals residing in the Santa Clara County. Position Responsibilities include:
Supportive Services
- Develop individualized treatment plans in collaboration with tenants which address identified needs and short-term and long-term goals.
- Provide strengths-based case management and service coordination services designed to assist clients in
obtaining and maintaining stable housing.
- Provide ongoing and complete case management services necessary to assist tenants in reaching all their
treatment goals including, but not limited to: Street Outreach, Case Management, Mental Health Services , Housing Location and Stabilization, Independent Living Skills, Community Integration, Employment Linkage , Benefits Establishment and Linkage to community providers for substance abuse and primary health care.Documentation
- Maintain documentation standards as set forth by the program contract, PATH policies, practices, and
procedures, and within all local, county, state, and federal guidelines: Complete intakes, assessments, and treatment plans within the documentation standards set forth by the program contract, Medi-Cal, and PATH.
- Maintain documentation standards as set forth by Medi-Cal and PATH policies.
- In collaboration with supervisor complete mental health assessment and establish mental health diagnosis for program’s clients
- Complete progress notes on every face-to-face or telephone contact with tenant or collateral contact
by the next business dayWHAT YOU BRING

- Ability to work effectively both independently and within a team environment.PREFERRED QUALIFICATIONS
- Bachelor’s Degree in Social Work.
- Experience working in homeless services, mental health, substance use and co-occurring disorders, and/or
with vulnerable populations providing intensive case management services.
- Substance Abuse Certification.
- Permanent Supportive Housing experience.
- HMIS experience.
- Medi-Cal documentation experience.MINIMUM QUALIFICATIONS
- Bachelor’s Degree in a human services field AND two (2) years of Medi-Cal billing experience.
OR
- An equivalent combination of comparable experience including professional experienceMANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Provide proof of full COVID-19 vaccination
- Successfully complete the following as a condition of hire:Tuberculosis Test

- Have reliable transportation
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
