Able to maintain a fluent conversation on a number of various everyday subjects, and discuss cases involving the field of work.Able to follow a discussion in Welsh, in Clear Welsh, on familiar matters relating to the post. Able to contribute to the conversation and answer questions.

Reading and Understanding - Intermediate Level

Purpose of the post

  • Ensure that the people of Gwynedd are at the heart of everything we do.
  • Help individuals to live their lives as they wish to live.
  • Take on the full care role in the home.
  • Collaborate with other agencies, for the benefit of the individuals, including attending reviews, meetings with health, consulting with specialists.
  • Administration of medicines

Responsibility for functions

  • Ensure appropriate use of users' property e.g., medicine and cash.
  • Make appropriate use of specialist equipment to assist the individual to live his/her life as they wish to live. This includes reporting on deficiencies and following practice protocols and specialist training as required.

Main duties

SUPPORT THE INDIVIDUALS WHO USE THE SERVICE

Role - full charge of home role if appropriate

  • Take responsibility for the residential care home and the responsible person role.
  • Lead staff with their work duties.
  • Take decisions in the absence of managers, e.g., receive admissions back from hospital, lead in an emergency, respond to enquiries.
  • Lead and respond to an emergency situation involving users and the building
  • Identify changes in the health and well-being of individuals and respond quickly and appropriately to them.
  • Be progressive and willing to make informed decisions independently, while also applying professional opinion and knowing when to attract support from the wider team and other professions.

Care Worker Duties

  • Excel in building relationships with people to understand exactly what would make a difference to them in their lives and empower the voice of the individual at every opportunity.
  • Be innovative and creative when giving care in a way that fully includes people, that continuously strives to include people in even the simplest of activities and enable or re-able people to take control of their own lives.
  • Be a key worker for individuals attending meetings and contribute orally and in writing
  • Maintain and support the procedure for administering and distributing medicines in accordance with the service policy.
  • Promoting the rights of the individual and encourage them to take positive and informed risks.
  • Follow an ethos that emphasises the importance of working with individuals and promoting their independence.   • Develop and follow personal care plans created in partnership with the individual, to promote and encourage him/her to live as full a life as possible by making use of any resources and activities available in the community.
  • Contributing to the process of assessing, monitoring, planning and implementing activities in accordance with the needs of the service user. These activities may include participating in programmes, exercises and specific interventions.
  • Encourage and support service users to develop their social skills to enable them to live successfully within the community, by setting objectives, problem-solving and learning how to identify when there is a change in circumstances and needs.
  • Test new ways of supporting and think outside the box for creative answers that draw from the individual's strengths, assets and informal network
  • Contribute to multidisciplinary talks about the personal well-being outcomes of people and ways they can be supported to achieve these, helping them to help themselves and use all the available resources in this field.
  • Working with people to assist and note barriers that prevent them from achieving their outcomes, and use information to discuss with the wider team, implement change and learn from experience.
  • Be responsible for maintaining all the administrative work in accordance with the guidelines set by the Service Support Manager on behalf of Cyngor Gwynedd.
  • Support with housekeeping duties, preparation of light meals
  • Operate and maintain IT systems.

COMMUNICATION AND RELATIONSHIPS

  • Work in partnership with colleagues in the health and care field, including social workers, community psychiatric nurses, occupational therapists and physiotherapists, GPs and 3rd sector partners.
  • Act on opportunities to build lives that are linked to family and community networks and be clear regarding what they are there to assist the person to achieve.

TRAINING AND SUPERVISION

  • Responsibility for self-development
  • A commitment to follow his/her own training programme in accordance with the National Requirements and accept responsibility for continuous professional development.
  • Have a mindset that is open to learn continuously and the desire to develop professionally within the health and care field.
  • Attend and contribute to constructive activities such as supervision and team meetings that support the mindset of improving and developing the service and staff well-being.
  • Register with the Care Council for Wales and behave in a principled way in accordance with the professional code of practice for social care.

  • Responsible for reporting any concern or suspicion that a child or vulnerable adult is being abused

  • Be responsible for managing information in accordance with the Council’s information management standards and guidelines. Ensure that personal information is treated in accordance with Data Protection legislation.
  • Ensure compliance with Health and Safety regulations in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.
  • Operate within the Council’s equal opportunities and equality policies.
  • Fully comply with Cyngor Gwynedd's Code of Practice for Medicines, including administering medicines.

  • Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s carbon footprint.
  • Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.

Special circumstances

  • There will be a need to go through the DBS processes
  • The need to work unsociable hours over seven days on a rota basis. This may include bank holidays and weekends as required.
  • Expected to work across other service sites if necessary.
