TRANSITION SUPPORT WORKER - Lincolnshire NEST Youth Housing Accomodation
Job Type - Permanent/Part-Time
Hours - 18.5 hrs per week (3-week rolling rota; 4 shifts on, 4 shifts off 5.45pm -11pm)
Salary - £10,632.41 per annum (inclusing £1300 Unsociable Hours allowance pro-rata)
Location - Grantham

Our services in Lincolnshire help vulnerable young people, who are homeless or are threatened with homelessness, to live independently. Our aim is to inspire the young people we support to be ambitious and embrace new opportunities.
This role sits within our Young People Hub in Grantham and it involves supporting young people aged 16- 25 to transition from supported accommodation into more independent accommodation.
Key Responsibilities
- To have an ongoing oversight of the exit date from service for each young person at the point of their move in and communicate regularly with the team to pre-empt when a young person is ready to move out of service.
- Acting as the first point of contact for the young people within the transition period to move on, phase and offer ad-hoc advice, guidance and reassurance.
- To support the young person to apply for move on options, such as other supported accommodation, council or private let properties.
- Support the young person to arrange and attend any viewings of move on properties with the young person.
- Complete necessary budgeting work, depending on the needs of the young person, based on realistic expenses and costs involved with setting up and running a home independently.
- Review and update the young person’s move on plan and complete regular joint meetings with the keyworker and any other external agencies involved.
- Following the young person moving into their new home you will complete home visits to offer any further support required.
- Host weekly drop ins to ensure access to support for those who have left service.

- An understanding of the welfare benefits system.
- Previous experience of working in Housing Management.
