Job title: Young Persons Support Worker

Project/Base: Sandwell

Reports to: Placement Manager

DAYS/Shifts: Late Sleep-in: 3-10pm - Sleep - 7-9am.

24/48 Hour shifts

W/ends: Early 8-3pm. Late 3 -10pm

24hr Shifts also available

HOURS: Various

Accompanied Care services:

Accompanied care is a 24 hour supported accommodation home for young people between the ages of 16-18 years. The project aims to enhance the independence skills and increase the personal, educational and social/emotional development of young people leaving care. We aim to engage with our young people, support to redress inequalities, value their opinions and empower individuals to take action on issues affecting their lives, including health, education, unemployment and the environment, by developing positive skills/attitudes and a positive mindset.

Overall Purpose

To provide advice, advocacy and support to young people placed within the home with regards to developing independence and addressing specific issues such as family breakdown, physical and mental health issues, substance misuse or any other contributing factor that can lead to disengagement from services and possible risk of homelessness when they reach independence. To work within Accompanied Care policies and procedures (ECM,Health and Safety, Safeguarding e.t.c) upholding the values and ethos of the organisation.

Main Responsibilities

  • To work in partnership with allocated social workers and other professionals in order to achieve desired outcomes set out in a young person’s pathway plan
  • To assess the needs of young people, and plan and deliver programmes related to areas such as health, fitness,nutrition, smoking, drugs, relationships, bullying, self harm, housing, budgeting, personal safety and to promote independence skills.
  • To advocate and mediate on behalf of service users, as appropriate and required, and be available to accompany them to appointments and meetings.
  • To mentor and support individuals to encourage social inclusion;
  • To establish positive working relationships with young people and to provide weekly key working sessions to draw up outcome focused Support Plans.
  • To be aware of safeguarding procedures and immediately report any issues/concerns to senior management and relevant partner agencies
  • To promote and support positive contact between young people and families where appropriate
  • To maintain accurate and up to date administrative and financial records and systems and assist in the compilation of monthly performance reports and other reports as required.

General

  • To undertake and participate constructively in induction, regular supervision, appraisal and relevant training, and contribute positively to good team relationships and continuous improvement and development of services.
  • To maintain confidentiality in line with organisational policy in relation to service users, staff and business sensitive information.
  • Requirements:
  • Minimum Level 3 Qualification in the Care of Young People.
  • Minimum of one years relevant experience working with young people
  • Relevant training such as Safeguarding, De-escalation Techniques, CSE, CCE, Dealing with challenging Behaviour, Missing from Care procedures.
  • Online DBS on update Service.
