The post holder will play a key role in supporting the All Age Continuing Health Care service across the region.

Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.

The post holder will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures, ensuring the National Framework for Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.

The post holder should hold a full UK driving licence and have access to a vehicle to fulfil the requirements of this post.

Currently we are looking for a motivated and experienced RMN, RGN OR ALLIED HEALTH PROFESSIONAL who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.

Our hours of work are Monday -Friday, 9am -5pm with the opportunity for agile working. An exciting opportunity has arisen within our Continuing Health Care Teams. Role involves assessing patients for Continuing Care funding (CHC), holding a caseload and reviewing patients as necessary. Good induction and training offered. Friendly Multi-disciplinary Team. The role is varied and involves working closely with Local Authority, Acute Trusts and with providers

You will work within the Mental Capacity Act and gain experience involving Best Interest decisions and Court of Protection work.
