The post holder will play a key role in supporting the All Age Continuing Health Care service across the region.
Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.
The post holder will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures, ensuring the National Framework for Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.
The post holder should hold a full UK driving licence and have access to a vehicle to fulfil the requirements of this post.
Currently we are looking for a motivated and experienced RMN, RGN OR ALLIED HEALTH PROFESSIONAL who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.
Our hours of work are Monday -Friday, 9am -5pm with the opportunity for agile working. An exciting opportunity has arisen within our Continuing Health Care Teams. Role involves assessing patients for Continuing Care funding (CHC), holding a caseload and reviewing patients as necessary. Good induction and training offered. Friendly Multi-disciplinary Team. The role is varied and involves working closely with Local Authority, Acute Trusts and with providers
You will work within the Mental Capacity Act and gain experience involving Best Interest decisions and Court of Protection work.

- Contribute to maintenance and monitoring of service standards around quality
- Supporting the Continuing Healthcare team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery
- Keyboard skills
- Access to vehicle for occasional travel to visit individuals, providers and other sites

- Ensure the principles and duties of safeguarding are holistically and consistently applied
- Supporting the arrangement of services required to meet the needs of individuals who qualify for NHS Continuing Healthcare and for the healthcare element of a joint care package
- Fostering transparency and a person-centred approach with patients, families and representatives
- Ensure the principles and duties of safeguarding are holistically, consistently and contentiously applied
- Working within a team of Continuing Healthcare Assessors, ensuring a high-quality service is delivered, supporting staff to achieve continuous improvement.
- Ensuring Continuing Healthcare guidance is followed and services are delivered to maximise patient safety
- Actively supporting the Continuing Healthcare team in working towards goals and objectives
- Contribute to maintenance and monitoring of service standards around quality
- Comply with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies
- Knowledge of Personal Health Budget process
- Mentoring and supervision of students
- Providing accurate and legible recording of information
- Ensuring data systems are kept up to date and maintained in line with standard operating policy and procedures
- Comply with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies
- Assisting in the facilitating of multi-disciplinary assessments and patient reviews in line with standard operating procedures
- Supporting the Senior Practitioner to ensure that all processes implemented are compliant with the NHS Continuing Healthcare National Framework
- Assisting in the management of cases as directed by the Senior Practitioner and the Team Lead
- Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines
- The post holder will be responsible for ensuring their mandatory training is up-to-date
- The post holder will be required to undertake any other duties and responsibilities within the framework of the post.
- The post holder will be expected to keep up to date with local, regional, national and international developments both within the NHS and Social Care and externally.
