Do you have a background of working in assessment and in a care home environment?
Berkshire Care Association (BCA) in partnership with Frimley ICS is piloting an exciting project to improve the discharge process for patients transferring to residential and nursing homes following an admission to Frimley Park or Wexham Park Hospitals.
We are looking to appoint motivated people for the Trusted Assessors roles to aid the discharge of people assessed as medically fit back to their care home.
Lead Trusted Assessor: £41 – 48k pa (NHS Band 7, pro rata to contracted hours). This would be a nurse, social worker or occupational therapist with good organisational skills and management experience.
These posts are initially for a 6-month period with the potential to extend subject to funding and outcomes of the pilot.
You will need:
· Strong communication skills
· Advanced clinical assessment skills
· The ability to think quickly and creatively
· Ability to professionally challenge
You will:
· Liaise with a wide multi-disciplinary team
· Complete a comprehensive assessment of the individual
· Provide support and assurance to providers
· Thus, enabling people to get back to their care home in a timely manner
Being a car driver and having access to a car will be essential.
