The successful applicant will work within the physical health department at at AFS Edenfield centre and Low secure services.
We provide a range of physical health interventions i.e. Wound care, ECGs, Venepuncture, Clozapine monitoring.
Health promotions such as Dietary and weight management, smoking cessation, Sexual health, Men's and women's health.
You will work along side visiting professionals such as GPs, Optician's, Podiatry and also the ward based mental health team.
The Trainee Assistant Practitioner will deliver high level, quality care and evidence-based, physical health interventions within a multi-disciplinary team. The Trainee Assistant Practitioner will provide planned care and be able to take appropriate action to achieve the planned aims and objectives.
The role of the Assistant Practitioner is to work with the multi professional team and the Registered Nurses to assess, plan, organise and deliver clinical care in line with the service pathways and as agreed with supervising clinician(s).
Support and co- ordinate the physical health department in care delivery,
Assess the health and well-being needs of service users on a caseload, as directed by the relevant professional.
Plan, deliver and evaluate programmes to address people’s health and well-being needs within the scope of a prescribed plan of care from a relevant professional.
