POST TITLE: TEAM LEADER

RESPONSIBLE TO: Registered Manager

REPORTING TO: Manager

JOB PURPOSE: To support other members of staff in the provision of care for young people; ensuring the young people retain their dignity and individuality, and to encourage and enhance their self esteem and quality of life, acting as a team leader as required and being involved in the general activities of the Home.

SKILLS, KNOWLEDGE & QUALIFICATIONS

Essential:

* Good communication, teamwork and leadership skills

* Willingness to undertake training

* Full UK driving licence, or working towards gaining full driving licence

* Diploma level 3

Desired:

* Experience of working in caring environment with young people

* Experience of working with mental health service-users

Responsible to: Home Manager/Assistant Manager/Manager

JOB SUMMARY

1. To assist the Home Manager & Assistant Manager/Manager in managing a staffed residential home for young people with specific needs.

2. To help promote a stimulating, caring and culturally appropriate environment for the Service Providers.

To provide practical and emotional support to enable

Service Users under the guidance of the Management team.

3. To work closely with Service Users and their families according to programmes and plans that have been implemented.

4. To deputise in the absence of the Home Manager and Assistant Manager/Manager

5. To lead shifts when most senior person on duty or when directed to do so by the Home Manager or Assistant Manager.

MAIN RESPONSIBILITIES AND DUTIES:

General Duties:

Supervision Support Workers

a.) Being responsible for anticipating Service User’s needs and communicating these needs to other staff and professionals involved.

b.) Writing review reports and other reports as necessary.

c.) Being involved in review meetings concerning Service Users.

d.) Being involved in setting up and implementing personal care plans for Service Users.

e.) To be involved in the selection, assessment, and admission of new Service Users with the Manager/Assistant Manager.

f.) To help new Service Users settle into the home.

To participate in the full range of caring duties including:

g.) Helping Service Users with leisure interests and skill development inside and outside the home.

h.) Domestic duties (washing, cleaning, cooking etc)

i.) Listening to Service Users, including their feelings about any difficulties they may be having.

j.) Providing personal care to Service Users enabling them to look clean and attractive.

k.) Helping Service Users to keep their possessions and living areas clean, tidy and safe.

l.) Helping Service Users to make informed choices.

m.) Accompanying Service Users on holidays.

n.) Implementing personal care plans set out for Service Users.

o.) Assisting with day care activities.

p.) Communicating with other staff about Service Users’ needs/activities.

q.) Recording Service User’s needs/activities/behaviours in the home’s diary system.

r.) Facilitating regular Service User meetings.

s.) To help ensure that each Service User’s health needs are met including:

- Liaising with medical services (e.g. GP, Dentist)

- Ensuring the implementation of appropriate medication procedures.

To help Service Users express their needs and have these met:

t.) Implementation of individual programme plans and their on going development.

u.) Establishing good relationships with Service Users’ relatives and advocates.

v.) Co-ordinating reviews at least once a year for each Service User.

w.) Ensuring up to date records are kept in Service Users files.

Liaison and Administration

x.) To establish a good relationship with Service Users, friends, volunteers and advocates.

y.) To work closely and in a professional manner with other professionals, both within and outside

z.) the Home e.g. GPs, Consultants, Social Workers.

aa.) To use communication systems effectively e.g. diary, log book etc, and verbally to colleagues.

Team

bb.) To participate in staff meetings, training courses and supervision sessions.

cc.) To be a constructive member of the staff team, e.g. to work to the guidance of the Home

dd.)Team Leader to work co-operatively with fellow workers and to use own initiative.

Other Duties

 To undertake any delegated tasks from the Manager such as weekly checks and monitoring

 To support the Manager during recruitment and induction programmes

 To carry out Supervision of junior staff as directed by the Home Manager.

 To participate in training as required.

 To act in accordance with all policies, e.g. Health and Safety, Admission

Procedures, Fire Drill etc.

 To work in a way consistent with the principles of ‘normalisation’ and ‘Equal Opportunities’ i.e.

to give each Service User practical skills and social status that are valued within their

communities. To have anti-racist and anti-sexist work practices, which help Service Users value

anti-discriminatory attitudes and behaviour.

 Driving the home’s transport, if you have a clean driving licence.

 Such other duties as may be required.

HOURS AND ANNUAL LEAVE

 A flexible 24-hour rota system is worked including evenings and weekends.

 Holiday entitlement: 28 days holiday per year including statutory Bank Holidays.

MAIN RESPONSIBILITIES:

1. Promote and ensure the good reputation of the Home. To maintain and demonstrate a positive attitude towards young people, their families, staff, visitors and others.

2. To engage as a named support worker for one or more young person, assisting trained staff in providing care.

3. Under the supervision of the Assistant Manager or Registered Manager, assist in the development of care plans for young people.

4. Under the supervision of the Assistant Manager or Registered Manager, assist in the preparation of review reports for the young people, participating in the feedback at review meetings as required with support and supervision.

5. Undertake Team Leader status within the Home as required, reportable to the Assis tant Manager and/or Registered Manager.

6. Ensure the safe and effective handover of all relevant information pertaining to the young people, operational and administrative running of the Home to the next team of staff.

7. Deliver identified training to junior staff under support and supervision.

8. Support young people who need assistance with independent living skills/personal hygiene and encouraging independence.

9. To demonstrate non-discriminatory practice in all aspects of work.

10. Assist young people who have restricted cognitive abilities in achieving optimum activities of daily living.

11. Carry out any levels of observation and/or supervision as directed by the Assistant Manager or Registered Manager.

12. Under the supervision and guidance of Assistant Manager or Registered Manager, report on the well-being of young people.

13. Update self with all priority need care plans of young people and complete written daily records as instructed by senior staff.

14. To support the development of the process, practices and delivery of the young person’s care and progress.

15. Participate in the framework of social activities within the Home by interacting with young people and helping them with meaningful activities and interests.

16. Respond to enquiries, answer the door and telephone appropriately and professionally.

17. Carry out regular checks on young people at intervals determined by the individual care plan.

18. Closely monitor young people who may be distressed and/or present with challenging behaviour due to emotional, intellectual or mental health needs, reporting any changes to the Assisstant Manager or Registered Manager.

19. Make all visitors to the Home welcome and provide refreshments and any assistance they may require.

20. Support/encourage young people to attend to household chores as planned in the therapeutic programme.

21. Escort young people during planned trips and leave from the Home as well as on social outings.

22. Assist in the serving of meals and drinks as requested/required.

19. When not attending to young people’s needs use the time constructively in undertaking domestic duties as requested/required, updating self with knowledge of care plans and/or other essential updates as directed by the Assisstant Manager or Registered Manager.

20. Report immediately to the Assisstant Manager or Registered Manager any clinical incident or accident occurring within the Home.

21. Report immediately to the Assisstant Manager or Registered Manager any illness of an infectious nature or accident incurred by a client, colleague, self or an other.

22. Notify the Assisstant Manager or Registered Manager as soon as possible of the inability to work, and also on return to work, from all periods of absence.

23. Maintain complete confidentiality of all matters concerning the Home, young people, staff and related work.

24. To assist in ensuring quality targets are met in accordance with agreed standards.

24. Participate in staff and young people meetings.

25. Attend mandatory training sessions in conflict management including physical interventions, first aid, manual handling, health and safety, fire safety, food handling, infection control, child protection and others identified by the Registered Manager as required.

26. To participate in agreed systems of supervision, appraisal and individual performance review as an employee.

27. Understand and ensure the implementation of the Home’s health and safety policy, emergency and fire procedures.

28. Report any faulty appliances, damaged furniture or equipment and any potential hazard to the Assisstant Manager or Registered Manager.

29. Ensure the security of the Home is maintained at all times.

30. Adhere to all Nirmal Homes policies and procedures within the defined time scales, Ofsted/standards and guidelines, Department of Health guidelines and legislation, including the Mental Health Act and Code of Practice.

31. Undertake other duties, as required, by a Assistant Manager or Registered Manager.
