Job description
We are seeking an individual to join our Community Rehabilitation Team based Parkview Care Centre Carnoustie. The post holder will work across services including Community Hospital In-Patients, Domiciliary, Early Supported Discharge (ESD), Prevention of Admission (POA) and Enhanced Community Support (ECS). This post will involve mainly Physiotherapy duties but may also include some Occupational Therapy duties. You will work closely with the Physiotherapy & Occupational Therapy teams and within the multi-disciplinary team. You will have the ability to liaise effectively with the multi-disciplinary and multi-agency team within the locality and will be able to work independently. It is desirable that you have worked previously as an AHP PT/OT support worker at Band 3 and have experience working with older people in a clinical setting would be advantageous.
