VIRTAUL WARD - COMMUNITY NURSE

As a Community Nurse at the Virtual Ward you will be expected to manage daily operations of caseload, triage referrals for admission, support staff with patient escalation. Troubleshoot, safety net and make clinical decisions to keep patients safe at home. In addition, you will further develop junior staff.
Please note this vacancy role requires face to face patient management as well as virtual management and therefore the team members will be expected to conduct face to face assessments in line with patient/ caseload needs.
To provide high quality nursing care and participate in the assessment, planning, implementation and evaluation of the care needs for the patient group.
To deliver compassionate person-centred care, and treat patients, service users, their families and their carers with consideration, dignity and the respect you would want for yourself or your family.
To provide relevant forms of care with minimal supervision and participate in the supervision and teaching and assessment of unregistered or junior staff and students.
To play an active part in the continuing development of the ward or department and contribute to the maintenance of an environment conducive to the delivery of high standards of patient care, by promoting good
communication and relationships between staff, patients, relatives and visitors.
