Job description
Patient/Client Care Assess and plan patients care needs in an accurate and timely way, implement and evaluate the planned care using a holistic and evidence based approach Identify any additional needs of the patient/carer and refer to appropriate member of the multi-agency/multi-professional team via the agreed pathways. Make reasonable adjustments to maximise the patient’s experience as need identified Responding to and taking a lead in emergency situations in the home To ensure continuity and co-ordination of care of patients moving between home and hospital Support and enable patients and carers to identify own health needs and strategies to improve and maintain wellbeing. Accurately observe and record patient’s clinical condition, and initiate appropriate action, in response to any change in condition.
