Salary: £23,348 per annum (sleep in payments and overtime additional)

You will have experience in working with people with learning disabilities and mental health support needs and have previously demonstrated strong key worker skills with proven ability to undertake the duties outline below.
We follow the ethos of Positive Behaviour Support, engaging individuals in active support and focussing on community participation. The main duties for this post involve leading on key working duties:
- Completing and reviewing person centred support plans
- Completing and reviewing risk assessments, including positive risk-taking interventions
- Holding person centred reviews – including the monitoring and reviewing of people’s individual goals and evidencing outcomes
- Monthly key worker reports to reflect progress of goals and the delivery of person centred support
- Working alongside the Positive Behaviour Support Lead to review PBS plan and interventions
- Supporting people to maintain their independence and wellbeing, as well as accessing community activities, offering personal care and support with daily living tasks
