This is an exciting opportunity to work as part of our “Outstanding Team” in the Special Needs School Nursing Team supporting children with complex health needs in Special Schools.
We are looking for Special Needs School Nurses to join our dynamic team. Hours are flexible to fit in with the team as well as ensure your work life balance.
Based out of The Oak Tree Centre in Huntingdon and The Peacock Centre in Cambridge, the team cover the 9 Special Schools in Cambridgeshire and Peterborough delivering quality care to make a difference to children and young people requiring nursing care in their homes/nurseries/school setting. Children benefit from the expertise of the hospital in the comfort and safety of their own environment.
You will be supported to develop your expertise both within your team and by regular reflection and professional training during the comprehensive induction period. The opportunity to progress and to undertake the degree in Special Practice Community Children's Nursing will be encouraged and supported.

You will be joining a team of highly skilled nurses who facilitate training and support to ensure that children with complex health needs are able to access school. In addition supporting families in complex case management and completing EHCP’s.
Experience of community work would be an advantage as well as experience, knowledge and understanding of working alongside other agencies and professionals which including education, health, social care and the voluntary sector.
You will be responsible for delivering a high standard of training, support and nursing input to the Cambridgeshire & Peterborough special schools. You should have excellent communication skills and be able to use your own initiative whilst also working well as a team player.
Due to the nature of the role it is essential that applicants are able to travel effectively and independently on a daily basis to and from the base, clients’ homes and other venues around the geographical area whilst transporting equipment.

Main Duties and Responsibilities
To assess the health needs and identify risks for CYP who meet eligibility for the service within the school setting in preparation for their admission to school
To plan, implement and evaluate individualised, holistic, and compassionate nursing care for CYP in a person and family centred way ensuring outcomes of care are agreed and meet the identified needs of the child and their family
To provide effective nursing care coordination for an identified caseload of CYP with complex physical health care needs, acting as ‘Lead Professional’ where identified in the CYP’s care plan
To provide active support in safeguarding, report writing, attending case conferences and core groups
To provide valuable health information to back up EHCP for identified children on caseload
To provide face to face and virtual training in specific health care tasks, e.g. enteral feeding; oxygen therapy, etc. for named individual children and assess competence of identified carers within the community
To be actively involved in promoting the Universal Offer in Special schools e.g. health promotion and parental/CYP training sessions
Attend multidisciplinary meetings and medicals for CYP with complex physical health care needs in school, providing nursing care reports as required
Confidently liaise with members of a multi-professional/ multi-agency team regarding clinical management of CYP to ensure effective treatment and co-ordinated care, leading decision making when appropriate
Ensure delivery of and advice on evidence based clinical care within the school and community environment
For non-medical prescribers (V300) and nurse prescribers (V100) to actively engage in prescribing role of the service. If you have a non-medical prescribing qualification, you will be expected to practice as a prescriber as part of this role
Promote the health, comfort, dignity, privacy and quality of life for CYP
Facilitate return school as soon as possible after discharge from both local and tertiary hospitals
Work within the limits of the NMC Code and guidelines, Trust policies, service operational policy and own professional competence
Ensure prescribed nursing care is carried out in line with local and national policies including, taking of blood samples or specimens as required, undertaking assessments for continence and developing person centred plans to promote continence for CYP
Provide skilled, evidenced based and sensitive nursing care to CYP. Practice in collaboration with family and other agencies to ensure good care assessment and delivery supporting the family with choice
Line management of staff
Communication
Complete accurate clinical records in line with local policy. Be aware of the requirements of both paper and electronic record keeping
Maintain excellent communication with the CYP and family on the caseload and as their advocate ensure that they have ease of contact with the team appropriate information and support. Records will reflect the CYP’s care, present condition and progress and be available to colleagues
Budgetary Responsibility
Be aware of budget requirements and role model the need for cost effective delivery of care
People Management
Receive regular clinical and safeguarding supervision in accordance with good practice guidelines and Trust policy
Train and develop colleagues and to carry out at least annually an appraisal, objective setting and personal development planning meeting
Maintaining own professional development and requirement to take part in appraisal and KSF process
Participate in recruitment, orientation and development of new staff joining the Children’s Community Specialist Nursing team
Research & Development Activity
Required to undertake R&D activityas required for service development
Clinical and Practice Governance
Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act
Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies
The post holder must adhere to the Trust risk assessment and risk management processes
The post holder must adhere to infection control policies and procedures
It is a condition of your employment that you are currently registered with NMC, and it is your responsibility to maintain your professional registration
Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust
The post holder must participate in clinical and other audits as required
Provide patients and their families /carers with information on standards they should expect from the team
Participate in clinical supervision on a regular basis
The post holder is required to participate in relevant emergency preparedness process for their team
