Job description
A local healthcare organization in Fremont is looking for a Social Worker with a Master's degree in Social Work (MSW) and at least one year of experience working with elderly or frail individuals for a contract to hire opportunity.
Cantonese Speaking is a huge plus.
The ideal candidate should have strong organizational and communication skills, be able to work well with diverse populations and within interdisciplinary teams, and have basic computer literacy. Bilingual ability in Cantonese may be required for certain assignments so a huge plus. Preferred qualifications include at least one year of post-MSW experience, one-year case management experience, and one-year healthcare-related experience. As a Social Worker , you will provide case management services, assessments, and care coordination for participants and their families to maintain their independence., you will provide case management services, assessments, and care coordination for participants and their families to maintain their independence.
Hours: 8am-430pm Monday to Friday, maybe some weekends or holidays as needed.
