A Social Worker is required to join a multi-disciplinary team based in the Lancashire area. The Social Worker will be responsible for working with individuals and families to promote their well-being, as well as to provide support and guidance to individuals and their families in a therapeutic environment. This role is based within a hospital setting.
Responsibilities
- Collaborate with other professionals to provide comprehensive care for clients
- Provide care and guidance to clients in a therapeutic environmentImplement interventions that promote client autonomy, ability to make decisions, and respect of individualized needs
- Monitor client’s progress and document needed changes in behavior, behavior modification, or treatment progress
- Maintain accurate records of services provided, including but not limited to, notes, progress notes, case reports, and care plans
- Attend professional meetings and keep updated on case information or developments that may affect the client’s treatment plan
- Represent the agency (including staff) in a positive manner at all times.
- Report concerns or issues regarding the client’s treatment plan to management.
- Work with other professionals as needed.
Experience:
- Experience working with individuals with mental health issues (preferred)
- Experience working with individuals with developmental disabilities (preferred)
- Experience working in a therapeutic environment (preferred)
- Bachelors Degree (preferred)
