Job Summary
The Nurse Home Visitor acts as a case manager and implements standard protocols for families enrolled in Nurse-Family Partnership. They will assess physical, emotional, social, and environmental needs of women and their families. Providing guidance and instructions to mothers and significant others regarding pregnancy, infants, and toddlers based on the standardized approach of Nurse-Family Partnership.
Job Duties
Provides overall coordination of care and home visitation for families enrolled in Nurse-Family Partnership.
Implements Telehealth per National service office guidelines and agency policy.
Assesses physical, emotional, social, and environmental needs of women and their families as they relate to the Nurse-Family Partnership domains.
Initiates a plan of visitation based on the standardized protocols of the Nurse-Family Partnership program.
Assists women and their families in establishing goals and outcomes.
Provides anticipatory guidance and instruction to mothers and significant others regarding pregnancy, infants and toddlers based on the standardized protocols of the Nurse-Family Partnership program.
Provides education, support, and referral resources in assisting women and their families in attaining their targeted goals.
Evaluates the effectiveness of the plan of visitation and the outcome of each visit.
Completes and submits all appropriate documentation forms within 48 hours.
Provides for discharge planning.
Educates referral sources and the public about Nurse-Family Partnership.
Develops and maintains community relationships to support client referrals.
Completes all required Nurse-Family Partnership education and orientation.
Develops therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
Participates in review and analysis of reports and other reports available through Nurse-Family Partnership for achievements and areas for improvement.

Position Details
Hours: Typical hours are 8 AM to 4:30 PM with the flexibility to work occasional evenings to accommodate client needs.
Coverage area: Union and Snyder Counties
Community-based: This role is primarily community based in nature. Primarily meet with participants in their homes or in-person in other settings in the community like a library, park, McDonalds, etc.
Preferred qualifications: Experience in Maternal/Child Health, Home Visiting, Behavioral Health, or Public Health
Education: Graduate from Specialty Training Program- Nursing, Bachelor’s Degree-Nursing (Required)
Experience: minimum of 2 years- Clinical (preferred)
Certifications(s) and License (s): Licensed Registered Nurse (Pennsylvania) – RN – State of Pennsylvania, Basic Life Support Certification
