Job description
To manage a complex caseload of service users as part of a hospital avoidance and discharge pathway and to co-ordinate care in the context of relevant health and social care legislation and policy. Frameworks including but not limited to: The Care Act 2014Mental Health Act 1983/2007Mental Capacity Act 2005Human Rights Act 1998Interagency Adult Safeguarding policies and proceduresCHC Framework/Guidance To contribute to the assessment and care management process within a multi- disciplinary team To make service users aware of financial contributions, to provide information enabling service users to make an informed choice.
