Job description
You will be a Social Work England registered Social Worker or Mental Health RMN and likely to have experience in CMHT, Early Intervention, Assertive Outreach, Crisis, Safeguarding or Adult Care Mental Health teams. You will have a minimum of 2 years post qualifying experience and proven continued professional development.
As Assessment Practitioner you will join the assessment arm of the Quality and Practice team of registered mental health professionals working in partnership with the NHS and local authorities across the country to support the discharge of people from low, medium and locked rehab services to recovery focussed community services in a unique partnership with other housing and development organisations.
Experience in screening referrals is preferred, but the ability to analyse and interpret multiple sources of background information, sometimes with conflicting information and produce clear and concise documents and reports is a must. This is a homebased role and the ability to travel and manage workload and time is key.
This role is non case holding and no on call.
Ideally you will be based along the M5 corridor which would allow for easy reach to our customers and clients who will eventually be residing in our schemes in Coventry, Redditch and Worcester as well as on occasion travel in the south west. As and when required, you will also travel across England to our other schemes.
You will agree timescales and workloads on a continual basis with the assessment manager and will travel at least regionally on a weekly / fortnightly basis with occasional overnights.
This role is subject to an enhanced DBS Disclosure.
