Job description
About the Transition Youth Worker role :
We are seeking a skilled and dynamic individual who can effectively manage a caseload for young people with complex life limiting conditions on their journey into early adulthood within the Transition Service. You will work alongside the wider teams at Hope House and Ty�Gobaith, and external care providers. You will empower the young people to have their own voice, and to enable them to have the confidence to navigate into the adult care, housing, education and benefits system.
