Locum Social Worker - Discharge to Assess
Temporary, Full Time
£192 per day PAYE / £207 per day Umbrella
Location: Colchester & Tendring

This is a chance for an experienced Social Worker to expand their knowledge and experience in working within a Discharge To Assess Model. This model of practice is ever evolving and developing and as a valued member within the team you will be an influencer of change and progression.
You will be supporting adults who have recently been discharged from hospital or supporting them to prevent an admission into hospital. . We work with the adult, their family and/or networks as well as providers and Health colleagues to promote recovery and maximise independence. In Discharge to Assess we champion the ‘Home First’ approach, practicing the Recovery Model, working collaboratively with our health colleagues to continue adult’s care and assessment out of hospital.
We are looking for 4 Social Workers to undertake focussed work with adults who have already been discharged into temporary care provisions, whether that be in a temporary residential placement, or a domiciliary setting. This will be focussed time-limited work, with the intention to assess adult’s ongoing needs and transfer over to our neighbourhood teams at the appropriate time. This role will cover Colchester and Tendring, working hours are Monday to Friday 09:00 – 17:30. The successful applicant will need to be able and available to visit adults face to face to undertake their assessments and support planning, however we are flexible around office and home working.
We offer a range of supportive measures to team members, including regular supervisions, weekly case discussion sessions and team catch-ups.

About You
To apply, you should be a qualified social worker with current Social Work England registration. You’ll bring significant experience of social work, modelling good practice and providing advice and guidance in complex situations. As a subject matter expert in relevant professional practice, you should have a comprehensive understanding of current legislation, regulations, and guidance. With experience of chairing meetings involving clients, family members, carers, and other multi-agency professionals, we’re looking for your ability to develop strong relationships with partner agencies and the local community to provide a supportive framework for vulnerable adults.
You’ll also bring:
- Evidence of continuing professional development and capability of practice in accordance with current professional standards.
- An in-depth understanding of the financial implications of commissioned care packages.
- The ability to make clear decisions, to highlight and limit potential risks associated with intervention
This role involves travelling around North Essex. A driving licence and car - or the ability to meet the mobility requirements of the role through other means - is essential.
We are looking for an experienced Social Work practitioner, with the ability to work autonomously and at pace. This is time-limited work and we liaise closely with our neighbourhood teams to pick up any outstanding or ongoing actions following our initial intervention.
