Derby City Council is looking to recruit a Social Worker on a fulltime permanent basis. You will deliver the very best service and outcomes to adults within the Learning Disability Assessment Team. As a team we support individuals with learning disabilities who may currently be living in a residential/nursing care home, supported living, independent hospital or with their family. You will be assessing and reviewing customer support packages with a view to promoting choice and control using person centered approaches. You will also be supporting some individuals to move from residential care to a life in the community providing advice and guidance.
We wish to recruit an energetic and motivated individual, with good communication skills who can work effectively with other agencies.
As a qualified Social Worker, you will complete assessments and support plans that are person centered. It is desirable that you have a good knowledge base and experience in supporting individuals who have a learning disability. You will need to be able to prioritise your work and respond to and support customers changing needs, whilst demonstrating empathy and adapting your communication to the individual’s needs. Applications are welcome from candidates with the ability to encourage, support and engage with carers and vulnerable adults.
Your ability to work with Commissioning Colleagues with a view to independent living will be desirable.
