The teams comprise of Social Workers and Occupational Therapists working to successfully support adults with a Long-Term Condition their families, carers, and representatives to live as independently as possible in the community.
What you can expect to be doing?
The role will involve undertaking comprehensive assessments focusing on the needs of Clients and Carers, supporting, and enabling people to minimise dependence and support independence through self-directed support, commissioning, monitoring and reviewing packages of care to meet individual assessed needs.
You will be working as part of an integrated multidisciplinary team delivering outcome-based services. You will be expected to demonstrate excellent communications skills, advocating for adults to assist them to achieve their outcomes, including signposting and providing information and advice.
Poole Town locality team is a supportive and busy team, committed to delivering quality services to the population of Poole. The successful candidate can expect to receive regular 1:1 supervision, full induction programme and continued training and development opportunities relevant to this job role.
About you
On a daily basis, good organisational skills and prioritisation to manage a varied individual caseload undertaking assessment, support planning and review work is required. The team undertakes a significant amount of Mental Capacity Work, Best Interest and MARM meetings as well as Court of Protection and Safeguarding Work. Therefore, a good understanding of these processes is beneficial however, training can be provided if required.
