Job description
We’re looking for a Social Work Assistant with an interest in developing their skills and knowledge in working with adults who have recently been discharged from hospital or recovering from a period of illness. If you want to support local people as they recover, and help them improve their independence, then this is the role for you.
You’ll join a supportive team of social workers and social work assistants to develop your assessment skills and application of the Care Act 2014 eligibility criteria. You’ll enjoy communicating with people to identify their strengths and needs and will ideally have experience of working with people in a health and social care capacity and will have completed a Diploma 3 or equivalent in supporting people.
This is a rewarding opportunity to support individuals holistically in collaboration with health professionals including Physiotherapists, Occupational Therapists and Rehabilitation and Reablement Assistants.
You will be joining a team and local authority who prides itself on the development of its people, and you’ll have various options available to support your progression including apprenticeship programmes.
This role is based in the community and includes assessing people in bedded units and in their own home.

Role Responsibilities- Carrying out strength’s-based assessments as outlined under the Care Act 2014 for service users and their carers.
- Supporting service users and carers where necessary in meeting agreed outcomes in the most resourceful way.
- Demonstrating an ability to use the city council’s email, social care data base and electronic recording system and any other systems deemed essential to the role.
- Being aware of quality standards and performance measures and ensure they are adhered to.
- Providing Practical Support where needed to individuals.
- Hold a Diploma 3 or above in Health and Social Care/Supporting People or equivalent.
- Experience working in a social/health care setting.
- Hold a full UK driving licence and have access to a car available for work.
- Good communication skills and able to understand individuals’ diverse needs.
- Can work as part of a team including working co-operatively with other professionals and agencies.
- An awareness of adult abuse and Mental Capacity.
- Ability to work autonomously but recognise when support is needed.
- Good written and recording skills
