Responsibilities:
About the role:
- Provide health and social care, under supervision from a Project Manager to clients across the cluster in line with professional and contractual requirements to include washing, dressing, toileting, monitoring conditions i.e. temperature, sterilising equipment and health checks. Providing advice to patients as agreed with qualified healthcare professional
- Initiate, complete and follow up on referrals to specialist care and report outcomes to Project Manager as required
- Liaise with and take direction from other healthcare professionals in order to provide the best possible specialist care for patients, ensuring coordination and continuity of care
- Clean equipment and other items, as required to ensure they are safe, serviceable and ready to use at all times
- Support resettlement process for clients being released and requiring ongoing social care
- Data input/case management via the use of IT systems including Word; Outlook; electronic case management systems etc
- Ensure that all safeguarding issues, including child protection and protection of vulnerable adults, are dealt with in accordance to CGL policies and procedures, and local and national statutory requirements
- Work within CGL’s professional development framework participating in active learning and developing professionally
- Utilise a flexible approach and where required participate in activities and tasks which directly respond to patient and service need
- IT literate, including proficiency in word processing and data entry
- Good interpersonal skills and a willingness to work flexibly as part of a team
- Able to respond to appropriate requests for assistance, maintaining confidentiality whenever necessary
- Good organisational skills
- Knowledge of safeguarding children and vulnerable adult’s procedures
- Be able to work with and manage challenging behaviour
- Completion of Care Certificate to commence within 12 weeks of starting employment
- Communicate confidently and effectively, verbally and in writing
- Show a capacity to work alone and the ability to keep calm under pressure
- Understand and have a commitment to the principles of equal opportunity and diversity
- Employ an empathetic and non-judgmental attitude toward patients
- Show commitment to facilitating positive outcomes for all patients
