Job description
A rare opportunity has arisen for a Shared Lives Assistant to join our small specialist Shared Lives Team.
Our team works to recruit members of the public as Shared Lives Carers, to enable vulnerable adults to live alongside the carer’s family in an ordinary domestic setting, in order for them to receive care and support and live an ordinary life. We specifically focus on supporting hospital discharge.
LIVE·WORK·SUPPORT
The Shared Lives service is mostly used by people with learning disabilities, head injuries and other physical disabilities, mobility impairment, users of mental health services and older people.
About the role
As a Shared Lives Assistant, you will work in partnership with care management teams, monitor the quality of existing arrangements, and offer sensitive support to Shared Lives Carers across Wiltshire to ensure the safety and well-being of vulnerable adults placed in their care.
You will take referrals from care managers, ensuring that sufficient information is gathered about the prospective customer to help determine the person’s suitability for shared lives. You will be giving guidance, advice, training, and constructive feedback in sometimes complex situations, ensuring the provision of a safe, effective, caring, and responsive service.
Working with the team to promote the service by talking to and providing information to the public and interested groups as part of the service’s development, will also form part of your role.
About you
Using a range of IT tools, you will require excellent writing skills to create and maintain accurate, concise, and relevant documentation.
You will possess a current driving licence, plus a willingness to travel regularly to locations across Wiltshire. The post is currently based in Devizes, but flexible working arrangements are possible to meet service needs.
