Summary: The Health Information Representative (HIR) is responsible for supporting the organizations documentation quality and compliance requirements, and maintaining the Electronic Medical Records (EMR) as well as the document management system on a timely basis.
Responsibilities:
Performs department process below. Review and monitor patient EMR documentation for accuracy and thoroughness following the requirements of the Medicare Administration Contractor (MAC).
Reviews ‘active’ and ‘discharged’ patient documentation daily. Identifies abnormal documentation and reports to the HIR Supervisor
Assures that all physicians signatures and dates are present on orders, certifications and recertification's.
Facilitates provider signatures when necessary, by presenting forms requiring signature to provider.
Scans and imports all hard copy patient medical information into the patients EMR in a timely manner.
Requests patient care documents from doctor’s offices, hospitals, and other healthcare partners.
Acts as a liaison to community providers needing to sign patient care documents.
Assists with departmental audits.
Provides patient care records once properly requested according to the Health Information Portability and Accountability Act (HIPAA) and CHI policies and procedures.
Obtains approval of collected records by the Quality and Compliance Manager/designee prior to releasing
Attendance at meetings and in-services as requested by the Quality and Compliance Manager.
Identifies errors easily and notifies the HIR Supervisor and/or Patient Care Manager (PCM) of any errors found.
Kindly communicates and assists employees when needed by providing appropriate work knowledge and guidance regarding EMR documentation requirements.
Maintain confidentiality and privacy policies at all times. Reports breaches per CHI policy.
Observe all safety CHI standards, policies and procedures.
Maintain a professional appearance of self and working area; exhibit appropriate behavior with co-workers, guests and clients: i.e., courteous, thoughtful and professional at all times.
Comprehensive knowledge of computer software and EMR system.
Qualifications
High School Diploma or GED.
One year of experience in healthcare billing or coding or use of Electronic Health records.
Medical terminology and hospice experience are desired.
Possession and maintenance of a valid California driver’s license and current automobile insurance. - comfortable with driving to other locations as needed.
Good verbal and written communication
Ability to type 35 WPM
Experience with Microsoft Word and Outlook is highly preferred.
Job Type: Full-time
Pay: $17.50 - $23.52 per hour
