Job description
The East Kennet Primary Care Network are seeking a Non-Clinical Patient Care Coordinator. The role will primarily be based at Kennet & Avon Medical Partnership, Marlborough and Ramsbury Surgery, Ramsbury.
Your role is to provide support to patients of all ages, their families and carers, signposting, supporting and navigating through available local services, for example sourcing care in the home or support to apply for jobs or benefits.
We welcome applicants with a range of experience and backgrounds and will provide training programmes tailored to your development needs.
The PCN wish to recruit two people to this role.
