Connect House is not your typical care home environment. We work in partnership with City Care to deliver reablement services for those who are medically fit for discharge from hospital but require further health input before moving on to their long term care solution. This could be returning home with a package of care or long term care services.
We are extremely passionate about the service we deliver and put high standards at the centre of everything we do. Our most recent external quality assurance audit awarded us an impressive 96%. We are currently awaiting our next CQC inspection and have high expectations.
We are seeking dynamic, motivated and dedicated individuals to join our leadership team in the role of Senior Health Care Assistant. This is a very important and valued role within the service. You will be:
- Responsible for leading a team of approximately 10 staff members deliver care for up to 37 individuals with varying health needs
- Be at the forefront of driving forward high standards and supporting those in your team adhere to the homes policies, procedures and best practice guidelines
- Be responsible for supporting citizens who use the service have a rewarding experience
- Supporting relatives to understand the reablement process
- Working alongside an MDT team including a Nursing Team, City Care Team including Physio, OT's, SALT & Dietician

The hours of working are 07.00 - 20.30 with an hour deductible for break.

A Level 3 in Health & Social Care or equivalent is desirable.
