Job Description.
My client, a London ICB requires an accomplished interim CHC assessor who can operate at a Band 7 level.
Day to day duties
- Deliver assessments of individual's eligibility for CHC and FNC
- Contribute to the clinical management function of CHC service
- Review checklist referrals, manage complex cases and sign off assessment ratifications
- Review care packages for existing patients
- Ensure that all care plans are in place and up to date
- Case load management
- Carry out Duty work as required
- Escalate issues with team performance
- Support junior staff when undertaking challenging assessments
- Provide clinical supervision and leadership as required
