About the Role:
We are looking for a self-employed Personal Care Assistant to provide support to a lady who has been diagnosed with kidney disease, Chronic Fatigue Syndrome, Autism Spectrum Disorder and has associated anxiety in relation to managing daily living tasks and communication with others. They need support to make complex decisions, to have control over their life and information tailored to meet their needs. The lady has a nephrostomy bag and she requires help and support with coping with the nephrostomy bag. The individual is expecting her first child in a few months. The lady has an assistance dog.
Hourly Rate – As a self-employed carer, please provide rates on application.
Day & Times – 21 hours per week, either 7 hours per day for 3 days per week or preferably, about 5 hours per day for 4 days per week. Actual days and times to be discussed.

Main responsibilities & duties
· Support to complete personal care – hair washing and ensure clean clothes.
· Support to maintain a healthy relationships.
· Support to manage personal relationships /sexual health /wellbeing.
· Support to prepare, plan meals, routine with mealtimes.
· Support to manage medication, order and prompts to ensure medication is taken.
· Support to maintain personal safety (around medication, harmful substances/equipment)
· Support to manage correspondence and take required actions.
· Support to manage financial matters and budget.
· Support to explore education and desired work goals.
· Support to attend college.
· Emotional reassurance at times of anxiety; especially, when attending medical appointments.
· Support to complete domestic tasks.
· Support to access the community – travel training is required for new places - use public transport more.
· Support to attend appointments/working with professionals. Making notes of what is said during the appointment.
· Support to plan time suitable leisure activities
· Support to build trusting relationships with support – individual struggles to engage with professionals and often disengages.
· Support to maintain/have personal & sexual relationships and supported to access further education around safe relationships.

§ experience and knowledge of the above conditions preferable but not essential

§ DBS Check will be required
