Here at Nottingham University Hospitals NHS Trust, we are looking for two new team members to join the CESR programme in Acute Medicine , providing an alternative route to specialist registration for doctors aspiring to be Consultants in Acute Medicine.

We have two posts available and we are open to applications from post CT doctors who wish to pursue a career in Acute Medicine and from more senior doctors who may have already started their CESR or are currently in a training programme but wish to pursue an alternative, single base route to CCT.

We offer robust and supportive educational and clinical supervision and mentorship, dedicated CPD time, study leave and study leave budget and access to all the rotations needed to complete your training (eg ED, Critical Care, HCOP). CESR Fellows will also have the opportunity to pursue a specialist interest – which may be clinical, research, service improvement or management based.


Main duties of the job

Under the supervision of a consultant, you will be required to assess and provide acute treatment to patients attending the Acute Medicine Department who are either referred from the community or the Emergency Department.

You will provide senior review to new admissions, assist in the supervision and training of junior doctors and other Allied Health Professionals, have an oversight of the take as well as expediting discharges and maintaining patient flow through the department.

We will ask you to participate in the general internal medicine registrar on-call rota associated with the specialty that you have rotated into.

The Acute Medicine department at NUH is one of the busiest in the country. Initially, you will join our middle grade rota, forming part of a highly motivated and ambitious multi-disciplinary team which includes medics, nursing staff, ACPs, Physician Associates and prescribing pharmacists. Detailed job description and main responsibilities

The Acute Medicine middle grade rota is largely split into three different roles:

  • B3 SpR – Responsible for the senior review of new admissions, support of junior doctors, undertaking of clinical procedures and having an overall oversight of the take as well as expediting discharges and maintaining patient flow through the department.
  • AMRA SpR – Responsible for the senior review of new GP admissions and Same Day Emergency Care patients. Support fellow junior doctors, undertake any clinical procedures and maintain patient flow through the department.
  • AMRAT SpR – Receive referrals to Acute Medicine from the Emergency Department to ensure that correct pathways are adhered to and patients are seen by the right person, in the right place and at the right time. This role also offers support for advice to our Emergency Department colleagues.
  • On-call duties cover both campuses and include GIM on-call, stroke thrombolysis, supporting the medical level 1 unit, and Level 1/Level 2 care for respiratory patients.

The responsibilities and duties of the post include:

  • To assess and provide acute treatment to patients attending the Acute Medicine Department who are either referred from the community or the Emergency Department.
  • Provide senior review to new admissions
  • Liaise with other specialties as indicated
  • Assist in the supervision and training of junior doctors and other Allied Health Professionals
  • Undertake clinical procedures including Lumbar Puncture, Ascitic Tap/drain, Pleural tap/drain, Central Venous Pressure Line insertion.
  • Have an oversight of the take as well as expediting discharges and maintaining patient flow through the department.
  • Provide Same Day Emergency Care (SDEC) to this cohort of patients.
  • Receive referrals to Acute Medicine from the Emergency Department to ensure that correct pathways are adhered to and patients are seen by the right person, in the right place and at the right time.
  • Provide admission advice and support to our Emergency Department colleagues.
  • Act as resuscitation team leader for seriously ill patients.
  • Participate in the general internal medicine registrar on-call rota or the rota associated with the specialty the post-holder has rotated into.

Training and Qualifications

Essential criteria

  • Full GMC Registration with current Licence to Practise
  • MRCP parts 1 and 2
  • MBBS (or equivalent)
  • ALS

Desirable criteria

  • Higher Professional Degree
  • Full MRCP
  • Formal teaching qualification

Experience

Essential criteria

  • Four years post-graduation experience with minimum one year experience in Acute Medicine
  • Twelve months experience at SHO or above level in Medicine in a NHS hospital setting
  • Able to manage a wide variety of acute medical emergencies

Desirable criteria

  • Experience in other specialties allied to Acute Medicine – e.g. ED, HCOP, Respiratory, Critical Care
  • Previous experience at SpR level both in hours and out of hours in a large hospital setting in General Medicine

Analytical and Judgement skills

Desirable criteria

  • Evidence of leading a QIP or Audit
