JOB TITLE
Remote Advanced Clinical Pharmacist Practitioner
SALARY RANGE
50k – 60k depending on experience and qualifications
MAIN PURPOSE AND SCOPE OF THE JOB
To provide remote (telephone) consultations to patients under the community pharmacy consultation service (CPCS)
To perform as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat a wide range of medical conditions presenting in primary care.
The employer Patient Health Care Ltd is working with a number of GP Surgeries around the country to provide this service to patients. The successful applicant will be able to work from home and will be provided with a lap top.

DUTIES AND KEY RESPONSIBILITIES
Summary
- Completion of CPCS consultations
- Undertake medication reviews
- Other reasonable clinical duties within competency
- Liaison with Pharmacy and Practice clinical teams as required in relation to patient management
- Taking part in training sessions for junior clinical pharmacists and other staff members as required under good profession practice
- Mentorship and training will be provided by a lead GP and lead Pharmacist
Specific Areas
- To work as an autonomous clinical practitioner accountable for own professional judgement and actions
- To consult, assess, diagnose, plan, implement and evaluate treatment/interventions and agree goals for patients presenting with an undifferentiated diagnosis through a coproduction approach
- To diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a patient management plan
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within the scope of practice
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Undertake telephone patient consultations
- Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care
- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
- Communicate with and support patients who are receiving difficult or complex information or results in a professionally appropriate and supportive manner

- Training may need to be undertaken outside of normal practice hours, and off site.
