Job description
This exciting new job role has been funded via the Stroke Quality Improvement for Rehabilitation (SQuIRE) initiative to support continuity of rehabilitative self-management approach for people in Newham who have had a stroke and require ongoing therapy. The posts will be based within the specialist Multidisciplinary team in Adult Community Neurological Service which includes Occupational Therapists, Physiotherapists, Speech and Language Therapists,, Psychologist, Rehabilitation Support Workers and administrators to support the team but will work closely across the stroke pathway in the community.
The Stroke RSW’s will work with patients in their home to continue to follow rehabilitation programmes, access third sector ad community resources
To visit clients in their own home and independently implement individual therapeutic programmes under the supervision of the team of therapists for each client in accordance with the agreed Care Plan and goal setting.
To assess for and prescribe minor aids and home adaptations for clients.
To maintain accurate and and up to date problem orientated documentation (multidisciplinary) for all of client caseload, consistent with department standards.
To independently evaluate progress of treatment and provide consistent feedback (verbal/written) to qualified practitioner up to date problem orientated documentation (multidisciplinary) for all of client caseload, consistent with department standards.
To adapt practice to meet individual patients’ circumstances, including due regard for cultural and linguistic differences.
As required be able to represent qualified practitioner at client related meetings and provide feedback regarding client treatment progress.

To independently manage and prioritise own complex caseload.
To deal with sensitive and emotive situations/information that may be distressing to individual clients using appropriate verbal communication skills
To work within Stroke Clinical guidelines and defined departmental policies.
To be able to receive and communicate messages from and to relevant staff and agencies.
To be able to act as point of reference for team and clients in regards to local community resources/services and to maintain up to date information regarding this.
To become the team administrator for SSNAP.
To identify and report issues surrounding safeguarding adults to line manager, complying with the organisation’s framework.
To attend multi-disciplinary meetings and/or case conferences to provide relevant information as part of the case-management process.
To assess any risk or potential hazards to the client or yourself before undertaking any tasks, including the initial Manual Handling risk assessment and report any concerns to a qualified staff member.
To communicate appropriately with clients and carers, taking account of beliefs and values. Ensure that clients are involved in the planning of their care through effective communication. Enable clients to find out about, and use, services and facilities to assist in maximising their functional ability through self-management.
To work closely with therapists in the Team and independently to modify and develop the individual therapeutic programmes to make them responsive to the changing needs of the client. This includes providing objective and subjective feedback from sessions with clients.
To work closely with other Rehab Support Workers and other agencies.
To maintain up to date and accurate case notes and write reports in line with the Trust and Department policies.
To adhere to Trust and departmental policies and procedures to protect the confidentiality of information about clients’ and colleagues at all times.
To share information with others, observing data protection guidelines.
To gather and update activity data accurately and regularly, ensuring the provision of such information promptly within local Trust guidelines.
To assist in defining and collating data for the purposes of service monitoring and clinical governance activities.
