NHS Greater Manchester Integrated Care - Bolton Locality Continuing Health Care Team has an exciting opportunities to join a committed and dedicated team of nurses. This is an excellent and exciting opportunity for experienced Band 6 to join our team and develop their skills as a NHS Individualised Nurse Assessor.
NHS Greater Manchester Integrated Care - Bolton Locality is looking for motivated nurse to join their Continuing Health Care Team. A registered nurse with experience and interest in community nursing or managing patients with complex needs.
You will be highly motivated, have the ability to work independently, but also as part of a team. You will have good interpersonal skills and give attention to detail.
The post will require the completion of detailed health needs assessments and reviews in line with current legislation and local policy regarding Continuing Health Care. There will be extensive liaison with clients, families, care providers and other professionals within the health and social care sectors. The post holder will also be required to provide case management for complex Continuing Health Care patients and their packages of care, making the role varied from day to day.
The successful candidates will need to be able to demonstrate the ability to complete comprehensive nursing assessments, have good written and verbal communication skills, must have the ability to work in stressful situations and be able to prioritise and organise their own workloads to meet deadlines.
As a registered nurse with experience and interest in community nursing or managing patients with complex needs. You will have experience and skills in case managing patients and carrying out assessments in various care environments.
