As a Reablement Worker with Brunelcare you will be part of the team in the short stay reablement centre, focusing on restoring a person’s independent function following an emergency hospital admission.

About the role
To holistically support individuals after a period in hospital with a reablement plan and work with them to find ways to meet their desired outcomes with a view to regaining as much independence as possible.
Liaising with health care professionals in order to review the progress of people utilizing the service and facilitate discharge home, where possible.
Participate and assist with any needs assessments, produce relevant Brunelcare documentation and complete weekly reviews of outcomes.
About you
Ability to work under your own initiative and be part of a close knit team.
Possess and demonstrate a competent and professional manner regardless of the situation.
A commitment to enabling people to live their lives as well as they can.
Previous healthcare experience would be an advantage.
