Job description
The post holder is required to hold and demonstrate positive values towards service users. It is expected that these values are demonstrated in the behaviour and performance of all staff.
The post holder must:
- Provide an excellent, professional and responsive service
- Protect and safeguard service users. This includes dealing with problems as they arise and speaking up against poor practice and abuse
- Contribute to the success of the person’s support by encouraging and motivating service users to meet outcomes in their support plans
- Contribute to the success of the service by being flexible, innovative and encouraging
- Work effectively and professionally with all stakeholders and partners
- In cases of emergency, take appropriate action by contacting emergency services, for example, doctor or ambulance and informing the Reablement Coordinators at the earliest opportunity
- To actively promote service user independence, choice and control in all daily living tasks.
- To actively participate in service reviews as required, reporting levels of functioning, improvements and concerns.
- To contribute to a daily written record of service users’ progress, ensuring that all records are accurate and a true reflection of the intervention.
- To support service users to maintain/improve links with friends, family, and other necessary community networks.
- To notify Reablement Coordinators of service users requiring assessment for other services.
- To encourage and assist service users to take medication as prescribed, within the guidance of the medication policy.
- To assist service users in preparing meals
- To give advice to service users on how to keep warm in the winter and to encourage service users to drink plenty of fluids during warm weather, in accordance with Government advice
- To complete all administrative records, such as timesheets, mileage forms and annual leave forms accurately and legibly and to submit them on time as required.
- To play an integral part in multi-disciplinary planning and casework.
- To participate in all reasonable activities at the request of the Reablement Service Manager.
- To actively work with all relevant professionals in the best interests of the service users who are being supported. These could include GPs, community nurses, OTs, physiotherapists and Speech & Language Therapists.
- To respect the confidentiality of the service user, all other staff members and other professionals that you come into contact with and the organisation at all times.

COVID-19 Vaccination
We continue to encourage all staff to ensure that they have been double vaccinated and received their booster. We recognise that taking the vaccine provides the best defence against COVID 19 for our patients, our staff and their families.
Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement – London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £3,898 and a maximum payment of £4,967 per annum pro rata).
