Introduction to the Role:
Crumpsall Vale Intermediate Care Unit is looking for a qualified Physiotherapist Therapist with experience in; to support with the delivery of care targeted at people who face unnecessary prolonged hospital stays or avoidable admissions to hospital, residential or continuing care. Introduction to the Trust, Ward & Team:
This unit is designed to provide a rehabilitation service with input from a multiciliary-team of dedicated health care professionals. It is a service which promotes faster recovery from illness, prevents unnecessary hospital admissions, supports timely discharges, and maximises independent living to its patients.
Responsibilities:

- Able to work autonomously and manage own caseload, seeking support when needed.
- Discharge planning as well as the ability to conduct discharge home visit assessments, preferably via use of a personal vehicle or alternatively via hospital transport.
- Attending and contributing to multidisciplinary weekly meetings in-order to deliver holistic treatment.
To be successful in applying for this role you will need:
HCPC Registration & Enhanced DBS.
- The ability to work under pressure and mange an independent caseload of roughly 8 patients over a 4-week period autonomously and offer support with additional caseloads if required.
- Understanding of different medical conditions such as; Multiple sclerosis, Parkinson’s disease, anxiety, and depression as well as dementia.
